Login Problems

  • Make sure your caps lock is not on.
  • Password is case sensitive.
  • Make sure you do not enter any spaces for the username or password.
  • If you have forgotten your password, click Forgot Password.
  • You will be locked out of your account after five unsuccessful login attempts. You will need to contact the Iris Help Desk at 601-579-5432 to regain access. You may also send us an email at iris.him@hattiesburgclinic.com.
  • Iris users are required to change their passwords every 6 months. You will automatically be prompted to do so at the appropriate time.

  • How to access a family member’s chart: For 0-17 year olds, parents or legal guardians must complete a proxy access form for each child at a Hattiesburg Clinic location or Forrest General Hospital’s Medical Records department. If you are requesting access to an adult’s Iris account, both parties must complete and sign a proxy access form at a Hattiesburg Clinic location or Forrest General Hospital’s Medical Records department.
  • How to obtain a copy of your medical records through Iris: After logging in, click the “My Medical Record” tab and choose “Request/Download Medical Records”. Follow the instructions given on the screen. Once your requested records are sent to your account, you will access these records from this same screen.
  • Have a question regarding Iris? You can log in and contact our Help Desk by sending Customer Service message. Click “Messaging” -> “Technical/Billing Assistance”. Please allow up to 2 days for a response.
