By your signature below, you acknowledge that you have thoroughly reviewed and understand the Terms and Conditions of using Iris. If you have any questions or concerns, please talk to a Hattiesburg Clinic/Forrest Health staff member about your questions before using Iris. By using Iris, including any links available through Iris, you confirm your understanding of this information, your acceptance of the risks and terms outlined below, and your agreement to abide by all of the terms and conditions of any linked sites as well as these Terms and Conditions.
All patients who use Iris are subject to these Terms and Conditions. When the term “you” is used below, it refers to the person who is authorized to access the patient’s medical information. In most cases this will be only the patient himself or herself, but adults will often have access to medical information for a minor child, and in some cases adults may have access to a Iris account for another adult due to disability or incapacity.
What This Site Offers
This online site called Iris is a service of Hattiesburg Clinic/Forrest Health, and is designed to offer health and medical information to Hattiesburg Clinic/Forrest Health patients. Health or medical information provided on Iris is not a replacement for proper medical attention. Always seek the advice of your health care provider with any questions you may have regarding medical symptoms or a medical condition. Never disregard professional medical advice or delay in seeking medical advice because of something you have read on Iris.
Iris is an Internet application that allows you to have online access to portions of your Hattiesburg Clinic/Forrest Health medical record and allows you to engage in certain online communications with your health care provider. Iris is not your official medical record. Iris is offered for the convenience of Hattiesburg Clinic/Forrest Health patients, and Hattiesburg Clinic/Forrest Health may discontinue the service at any time for any reason.
At this time we foresee that Iris will primarily be useful for the following types of activities:

• Receive selected lab results
• Communicate with your physician or other treating provider via secure messaging regarding health concerns
• Request prescription refills and prescription renewals
• View after visit summaries
• Links to other sites that will provide you with health information
Response to Electronic Communication
Hattiesburg Clinic/Forrest Health will make its best effort to provide a timely response to electronic inquiries. In some cases, the clinic staff who need to respond may not be immediately available so you should allow at least three (3) business days for a response. Emergency situations requiring immediate attention should not be submitted electronically.
Private Information on This Service
For information regarding the use, disclosure, and protection of personally identifiable information pertaining to Iris, see the Notice of Privacy Practices at www.hattiesburgclinic.com or at www.forrestgeneral.com. Hattiesburg Clinic/Forrest Health will send you a printed copy of the Notice of Privacy Practices upon request.

Appropriate Use and Termination of Use of Iris
You agree that you will use the electronic communication privileges of Iris in a courteous, respectful, efficient and informative manner, and you understand that this service is not for social dialogue. You are responsible for all information you send to Iris. You may not post or transmit any information that is illegal, threatening, libelous, defamatory, obscene, ethnically or racially offensive or discriminatory, profane, invades the privacy of another person or any other information or materials that constitute a crime, give rise to a civil action or otherwise violate any law or confidentiality agreement. Improper activities also include, but are not limited to, reserving appointment times that are not needed or are frequently cancelled and other activities that are deemed to cause a disruption to clinic operations in the sole discretion of Hattiesburg Clinic/Forrest Health. Hattiesburg Clinic/Forrest Health reserves the right to refuse to post any such message, to restrict, revoke or suspend your access to Iris, and/or to take any appropriate legal action related to any inappropriate use of the Iris system or the information it contains.
Hattiesburg Clinic/Forrest Health does not encourage or condone the copying and pasting or otherwise posting information from your Iris account into any other form of electronic communications, including but not limited to e-mail, social media sites, blogs, message boards or any other applications. Hattiesburg Clinic/Forrest Health will not be liable for improper disclosure of confidential information that results from such activity.
Either you or Hattiesburg Clinic/Forrest Health, at any time, may cancel participation in Iris. You have no entitlement to continued use of Iris. Hattiesburg Clinic/Forrest Health may cease your access to Iris at any time. You may contact Hattiesburg Clinic/Forrest Health at any time to request that your access to Iris be terminated.
Nature of Communications with Hattiesburg Clinic/Forrest Health
You understand that Iris will allow you to communicate directly with providers regarding your medical care. You agree that you will only use Iris to convey online messages of a non-urgent nature. You will only convey online messages that are suitable for communication in this medium. Some situations will be best handled either through a telephone conversation or an in-person visit. As described above, for any emergency conditions you should immediately go to your nearest emergency room or dial 911 immediately.
You also agree to communicate only through Iris for your own medical condition or for the medical condition of someone for whom you are authorized to make medical decisions. You agree that asking for advice on behalf of another person for whom you are not authorized to make medical decisions could potentially be harmful and is a violation of these Terms and Conditions.
Also, Hattiesburg Clinic/Forrest Health can only respond to the information you have provided. If the information you provide via Iris is incomplete or inaccurate, it will not be possible for Hattiesburg Clinic/Forrest Health to provide a complete or accurate response. You are responsible for any incomplete or inaccurate communications resulting from your submission of incomplete or inaccurate information to Hattiesburg Clinic/Forrest Health.
No Guarantee of Continuous Access to Iris
You understand that Iris may not be available to you at all times due to system failures, back-up procedures, maintenance or other causes. Access is provided on an “as-is, as-available” basis and Hattiesburg Clinic/Forrest Health does not guarantee that you will be able to access Iris at any particular time. Hattiesburg Clinic/Forrest Health cannot guarantee that use of Iris will be free of interruptions and is not liable for the inability to access or view the information contained on the site. No Guarantee of Completeness of Medical Record
Iris is designed for your convenience and enhances your access to certain parts of your medical record. However, Iris is not your complete medical record. Messages exchanged between you and your treating provider through Iris become part of your official medical record. This information is confidential but may typically be released with your consent or by legal requirement.
Authorized Access by Hattiesburg Clinic/Forrest Health
Any authorized person at Hattiesburg Clinic/Forrest Health who would otherwise be authorized to access your medical records may be authorized to view information submitted through your Iris account.
Your Responsibility to Review Iris Messages in a Timely Manner
Messages sent to you via Iris might contain information that is important to your health and medical care. It is your responsibility to monitor these messages in a timely manner. You are liable for any loss, injury or claims of any kind resulting from messages that you fail to read in a timely manner.
As a user of Iris you should be aware that you will be notified via e-mail when there is new medical information to be viewed on Iris. This means that any person with access to your e-mail will be able to see this notification. This could include your family members, employer or anyone else who can access your e-mail account. Although no private medical information will be sent, the notification that new medical information is available may be information that you would not want others to know. Thus, you should take this into account when providing an e-mail address or using Iris.
Once you activate an Iris account, it becomes the primary method to receive messages from Hattiesburg Clinic/Forrest Health providers who actively use Iris. In some cases, this information will be delivered only through Iris, and not by other means such as US mail. This may include, but is not limited to laboratory results, appointment reminders, and reminders for routine health checks.
Access to Another Patient’s Record
A patient may authorize other individuals to access the patient’s Iris account. For patients under the age of 18, access will only be granted to individuals with parental rights or legal guardianship. For all other age groups, please contact Hattiesburg Clinic/Forrest Health’s Health Information Management Department at (601) 264-6000 for more detailed information regarding the requirements for access to another person’s medical information.
If you have been granted access to another patient’s record on Iris and your legal relationship with the patient changes, you must inform Hattiesburg Clinic/Forrest Health immediately by sending a written notice to Hattiesburg Clinic/Forrest Health’s Health Information Management Department. Hattiesburg Clinic/Forrest Health reserves the right to revoke access to a patient’s Iris account at any time for any reason.
Access for and Use by Minors
Parents or legal guardians of a minor child under the age of 14 may request access to the child’s Iris account. Parents or legal guardians of a child ages 14 through 17 may be granted proxy access to the child’s Iris account with the consent of the child. A child age 14 through 17 may be granted access to Iris with the consent of a parent or legal guardian. Parents or legal guardians may contact the clinic where the minor is seen for more detailed information regarding the requirements for access to a minor child’s Iris account.
Under state and federal law, there are certain types of medical information that the parent or guardian of a minor patient may not view without the consent of the minor patient, and Iris access to this information may be denied. The posting of medical information is at the discretion of the patient’s individual physician(s ).

Iris information and the logos, icons and other marks shown or referenced within are reserved by Hattiesburg Clinic/Forrest Health and are protected by copyright and trademark law. You may access the content of Iris only for your personal, noncommercial use. If you desire to reproduce or download any part of the content for other than personal use, you must request and receive written permission from Hattiesburg Clinic/Forrest Health before doing so.
Changes to the Terms and Conditions
Hattiesburg Clinic/Forrest Health may modify or change these Terms and Conditions at any time. Such modifications or changes become effective immediately when they are posted to Iris. You agree to review these Terms and Conditions frequently so that you will be familiar with them. You further agree that each time you use Iris you are, by such use, consenting to the Terms and Conditions applicable at that time.
